When to Book a Home Visit
- New or worsening symptoms that concern you
- Difficulty traveling to a clinic or hospital
- Follow‑up after a recent hospital stay

If you ever feel your symptoms are severe or life‑threatening, go to your nearest urgent care center or hospital immediately
How to Prepare for Your Home Visit

- Write down any symptoms, questions or concerns
- Keep your ID and any medical records nearby
- Make sure we can access a clean, well‑lit area to assess you

Home devices like blood pressure machines help you and your doctor track your health between visits. Use them exactly as directed to get accurate readings.
Blood Pressure Monitors
- Sit and rest for at least 5 minutes before taking a reading
- Avoid caffeine, exercise and eating at least 30 minutes before checking
- Place the cuff on bare skin, not over clothing
- Keep your feet flat on the floor and your arm supported at heart level
- Record your readings with date and time
